This form must be filled out before any prop weapon or weapon‑appearance item is brought onto campus. Because items that look like weapons must be reported and approved in accordance with campus policy, this applies even if the item is inert or purely decorative. Please attach photographs, specifications, approval documentation, and any other relevant information that will help ensure approval of your request. Incomplete forms may lead to denial of the request.
